Federal Agencies that keep systems of records pertaining to individuals must post a "System of Records Notice" (SORN) in the Federal Register.
The SORN must explain how the individual can acquire these records and amend any mistakes, omissions etc..
The VA and Social Secuity Administration are examples of federal agencies with SORN's in the Federal Register.
Interestingly the VA has a System of Records containing only information from the Department of Defense pertaining to a Veteran.
Exactly what errors may exist will require obtaining the records, but as an example the VA uses at least 5 styles of Name for me. But the only name they should be using is the style of name used on the DD-214
In other words there are at least 5 separate groups of records pertaining to me based on my Name, and if they only gave your records for one style name then you are missing information from these other 4 record sets..
